Education Fair & Compliance

Safety Education and Annual Review for UWMC staff is recognized as an essential component of creating and maintaining a safe environment for employees, patients and visitors, to include equipment, building, grounds and compliance training.

Requirements:

  • All UWMC employees are required to complete this competency if it has been one year or more since you were hired or last completed the Education Fair.
  • Employees are required to turn in a completed copy of the current quizz to their supervisors as evidence of their completing the Education Fair.

2009 Education Fair and Compliance Annual Review

All UWMC staff, students and volunteers must complete the job specific components of our 2009 Education Fair, no later than 12/31/09. However, it is highly recommended that you complete this before annual review time. PLEASE PROVIDE THE QUIZ CONFIRMATION PAGES TO YOUR MANAGER AS EVIDENCE OF COMPLETION ! For your convenience, the education materials, quizzes and answer keys are now online at Catalyst and as PDF documents for printing!

We encourage all staff, students, and volunteers to take this learning on-line; however, if they are unable to access a computer you can print the slides, quiz, and answer key. We suggest you place these items in a binder and have them available for your staff as needed. The PDF documents that are ready for print are located above the catalyst links on this page.

 

The five 2009 Education Fair & Compliance Annual Review quizzes are:

  1. Cultural Diversity (Respect Through Understanding)- All staff must complete
  2. Workplace Safety (Safety Fair) - All staff must complete
  3. Organization Specific Topics - All staff must complete
  4. Compliance & Ethics Training - All staff must complete
  5. Clinical Topics - Clinical Staff must complete

Printing Instructions  

Important!! Do not print the entire quiz, the quiz can contain up to 30 pages of information.

Once the quiz has been submitted, please print the confirmation (also known as "Thank You") pages, which lists the quiz sections, your confirmation number, score, name, and the date. The 2 pages must be given to your manager as evidence of completion.  If you have questions about any of the information presented, please ask your manager.

How do I print the confirmation pages?

  1. Click File at the top of the confirmation (i.e. “Thank You”) pages
  2. Click Print Preview
  3. Click Print
  4. Select button in front of Pages
  5. Type 1 - 2
  6. Click Print

Confirmation Page

Did you know the catalyst tool can provide two types of confirmation pages?

Once the quiz has been submitted, please print the "Thank You" confirmation page, which lists your confirmation number, score, name, and the date.  If you need to print later you will notice the confirmation page looks different and is called a "Submission" confirmation page, which list your submission date, time and name.   Please be aware that the "Thank You" or the "Submission" page can be used as confirmation of completion.  

Where do I submit my confirmation page?

When completing a PAPER version of the quiz, please provide the original to your MANAGER and a copy to OD&T Box 359422. We appreciate receiving verification of the paper version quizzes because it will assist us when determing the completion rate. Please note that we already have a record of the electronic version and do not need an additional copy.
